How do I get to your office?
Our office is located directly inside the Orthopedic Center of Illinois in the south end of the building. If you come in the main entrance, please turn left and walk towards the end of hall. Our office is located in the last door on the right. Get directions on our Locations page.
What locations do you provide services?
We provide services throughout central Illinois including Lincoln, Carlinville and Taylorville.
What do I need to bring with me to my appointment
Please bring the following information:
- Insurance card(s)
- Driver’s license or Photo ID
- List of current medications
Do I need my medical records from another doctor or provider?
Yes, there are two different options:
- Contact your doctor or medical office, and sign a release form for their office to forward medical records to our office.
- Contact us and complete a release form for us to forward to the previous doctor.

How long will my appointment last?
Most appointments last approximately 30 minutes. However, appointments where patients receive other services could last up to three hours. Appointments may run late if surgery delays Dr. Butler. We appreciate your patience.

Will my eyes be dilated?
It depends on your type of visit; however, all patients seeing Dr. Butler for the first time will have their eyes dilated.
